Note for BOTH: If you are filling out the forms electronically, please open the forms in Adobe Acrobat (free download) to sign. It will not let you sign if you open it in a browser.
Medication Paperwork (all grades):
If your camper needs medication during the camp day (this includes OTC medications, epipens, inhalers, etc.) please make sure all necessary forms are filled out for the first day of camp, June 29th. You can also send ahead of time via email to: parks@lewisborony.gov.
