Required Paperwork (due by 4/11/25)
If you are filling out the forms electronically, please open the forms in Adobe Acrobat (free download) to sign. It will not let you sign if you open it in a browser.
If your camper needs medication during the camp day (this includes OTC medications, epipens, inhalers, etc.) please make sure all necessary forms are filled out for the first day of camp, June 30th. You can also send them to Katie ahead of time: recreation2@lewisborony.gov .
